在2021/9/14成功將台灣的疫苗注射證明轉成荷蘭的證明，相較其他歐盟國家，荷蘭算是開
放得比較寬，不僅EMA(歐盟醫藥管理局)同意的，WHO同意的也可以。
所以你的疫苗必須是下面幾種：
Astra Zeneca EU (Vaxzevria) ;
Astra Zeneca – Japan (Vaxzevria);
Astra Zeneca – Australia (Vaxzevria);
Astra Zeneca-SK Bio (Vaxzevria);
Pfizer-BioNTech COVID-19 Vaccine – United States of America;
Pfizer/BioNTech (Comirnaty);
Johnson & Johnson ((COVID-19 Vaccine) Janssen);
Moderna (Spikevax);
Serum Institute of India (Covishield);
Sinopharm BIBP(國藥);
Sinovac(科興).
同時你也必須要有BSN (Burgerservicenummer) 直譯為社會安全碼，類似身分證字號，
跟居留證號碼沒有直接關係。關於如何取得BSN，這是要去荷蘭留學或工作的人就會知道
的東西。
但如果你沒有要用疫苗護照，也可以單純申請這個入境荷蘭。
疫苗注射證明必須包含下列文件：

have been issued following vaccination with a vaccine that has been
approved by the European Medicines Agency (EMA) or is on the Emergency Use
Listing of the World Health Organization (WHO);
have been issued based on full vaccination. That means that:
the vaccination schedule consists of a single dose and this single
dose has been administered (Janssen); or
the vaccination schedule consists of two doses and:
both doses have been administered, or
one dose has been administered and it has been confirmed that the
person vaccinated had previously been infected with the virus SARS-CoV-2.
你必須要施打歐盟醫藥管理局(EMA)或WHOEUA通過的疫苗，而且除了嬌生施打一劑以外，
其他都必須施打兩劑，或是曾經得過武漢肺炎康復後並再施打一劑；
have been issued in one of the following languages:
Dutch
English
French
German
Italian
Portuguese
Spanish
必須為以下的語言：
荷蘭文、英文、法文、德文、義大利文、葡萄牙文、西班牙文。

information identifying the person who has been vaccinated; 足以辨識是誰施打
疫苗 e.g. 護照號碼
information demonstrating that the person in question has been fully
vaccinated against the virus SARS-CoV-2;
表示已經完整施打(看上面的條件)
the name of the vaccine and the name of the manufacturer or marketing
authorisation holder of each dose of the vaccine administered;
疫苗的名稱或核准輸入商
the date on which each dose of the vaccine was administered;
兩劑疫苗分別在何日施打
the name of the country in which the vaccine was administered;
兩劑疫苗分別在哪個國家施打
the name of the issuer of the proof of vaccination.
開立證明的人的簽名
社團上有人分享單靠小黃卡就得到，但我出國前覺得不夠保險，所以申請了一張英文診
斷書

這邊提供一個建議，如果你打的是AZ，記得後面加上「Vaxzevria」，讓GGD不會有疑問。
確定完備以上條件後，打電話到烏特勒支的GGD(類似衛生局的單位)預約(在荷蘭萬事都
要預約，沒預約他們就算坐在那邊喝咖啡也不會鳥你)，電話是：030-8002899。
電話中對方會跟你確認你的證明有沒有以上的資料，有的話跟他喬時間，他會寄一封
Email，還有48小時前會寄簡訊確認，然後提醒你過去要戴口罩。
到了烏特勒支車站，往南方出口，就是月台數字多的一邊(烏特勒支車站是荷蘭最大的車
站，總共有2X個月台)，右手邊出去就看到市政廳，外面有懸掛烏特勒支市旗。
進去以後到4樓還5樓(抱歉我忘了)的L櫃台，出示你的預約證明(EMAIL或簡訊)，會領到
其中一個櫃台，我到的櫃台看到我來了他才把口罩戴上(???
大約十分鐘，提供BSN、護照、上面那張證明，就換轉成兩張紙本文件。
其中一張會包含個人資訊，是在歐盟/申根/EEA區使用

另一張是荷蘭本土用的，然後當下人員會協助幫你在手機APP上認證，就會有QRcode可以
掃，並且嚴正表示紙本只發一次，效期一年，強烈建議把他掃描存檔起來，自己影印保存
也可以。

另外，因為台灣在白名單中，所以從台灣前往荷蘭(每個申根國家略有不同)，並不是必要
施打疫苗，可以以PCR(72小時有效)、抗原測試(24小時有效)登機。
