以下是詢問的內容和 Graduate Division 的回覆
Q1. 我應該要把 "Confidential Financial Statement" 和 "Sponsorship Information
Form" 寄到同一個地址嗎?

A1-1. Your file will remain in an Admission Pending status until we receive
your signed Sponsorship Information form and supporting bank statement(s).
You can email scanned copies of these documents to Cynthia Davis (copied
on this email) for review.

At the time of applying, you are only required to fill out the
Confidential Financial Statement, which is part of the online application.
If you indicated "Yes" on the Confidential Financial Statement to any of
the 3 sponsor questions, you would be requested to provide the Sponsorship
Information Form and supporting verification of support (original hard
copy documents) if you are offered admission and choose to accept.
The Confidential Financial Statement sections shows the required
verification of support you are required to provide for them term applied
for. If you are offered admission with fellowship, your offer letter would
indicate the required proof of support you would need to provide.
Q2. 繳交財力證明等相關文件的時候,是否有任何的 deadline ?
A2. While there is no specified deadline, you want to ensure that you give
enough time for our office to process and visa processing so providing us
with these required documents by May 2016 at the latest. Please note that
acceptable bank statements will need to be dated March 1, 2016 or if the
funds are frozen need to be frozen on or after March 1, 2016.
Q3. 我要怎麼知道學校已經收到我的相關文件? 在申請系統上可以查詢嗎?
A3. If approved, you will receive confirmation from the application system and
you will need to mail the originals to the address listed below.
Q4. 需要交 SIR Depodit 嗎?
A4. We do not require a SIR Deposit.
Q5. 除了上述的文件,我需要完成或繳交什麼東西嗎?
A5. Any missing items to complete your file will be addressed in your I-20
email which you will receive after we have received and approved your
provided verification of support.
